A transomental hernia through the greater or lesser omentum is rare, accounting for approximately 4% of internal hernias. Transomental hernias are generally reported in patients aged over fifty. In such instances, acquired transomental hernias are usual, are commonly iatrogenic, and result from surgical interventions or from trauma or peritoneal inflammation. In rare cases, such as the one described in this study, internal hernias through the greater or lesser omentum occur spontaneously as the result of senile atrophy without history of surgery, trauma, or inflammation. A transomental hernia has a high postoperative mortality rate of 30%, and emergency diagnosis and treatment are critical. We report a case of a spontaneous transomental hernia of the small intestine causing intestinal obstruction. An internal hernia with strangulation of the small bowel in the lesser sac was suspected from the image study. After an emergency laparotomy, a transomental hernia was diagnosed.

A glomus tumor is a very rare neoplasm consisting of cells that resemble the modified smooth muscle cells of normal glomus bodies. Here, we report a case of a 39-year-old male with multiple omental glomus tumors. The patient underwent a complete resection of the glomus tumors. This is a rare case of omental glomus tumors, and to our knowledge, this patient is the first with multiple omental glomus tumors to be described.

Actinomycosis is a chronic suppurative bacterial infection produced by Actinomyces israeli. The three major clinical presentation include the cervicofacial, thoracic, and abdominal regions. Abdominal actinomycosis is a rare entity which presents some difficulty in establishing a correct preoperative diagnosis. The diagnosis is usually based on histologic demonstration of sulfur granules in pus or surgically resected specimen. Recently, authors experienced a case of actinomycosis of the greater omentum in 38-years old woman. The patient underwent surgery under the impression of periappendiceal abscess. A 6cm sized firm mass was noted in the transverse colon. Histologically the mass was composed of fibroinflammatory mass with multiple actinomycosis granules.

Primary omental torsion is a rare condition and its etiology and pathogenesis remain unknown. It is very difficult even for general surgeon to differentiate this from other acute abdominal diseases, and so the most common preoperative diagnosis is acute appendicitis and the treatment of choice is resection of the affected organ. The authors experienced 7 cases of primary omental torsion at Holy Family Hospital since 1990 and review them retrospectively with other literatures. We conclude that omental torsion must be tried to find if there is serosanguious fluid or undescended omentum without pathology in peritoneal cavity during the emergency operation.

Actinomycosis is relatively rare infection which is produced by an anaerobic organism, actinomycetes, normally residing in the mouth, bowel and female genital tract. This disease is characterized by chronic inflammatory induration with abscess and multiple sinus formation. In most cases, the onset of this is preceded by an inflammatory or traumatic history on abdomen but diagnosis can't be made until operation. Recently this disease is treated with antimicrobial therapy and wide surgical excision. We experienced a case of omental actinomycosis and report it with brief review of literatures.
